Operations Leadership & Care Coordination (Multi-Site)

Practice Nursing in a Community You’ll Support and Lead

This is not a traditional RN role. This hybrid position blends hands-on patient care, people leadership, and clinic operations across multiple CareATC health centers.

What You’ll Do

  • Lead daily clinic operations, including workflows, scheduling, staffing coverage, and readiness
  • Provide people leadership through onboarding, coaching, engagement, and performance support
  • Ensure compliance with policies, procedures, and AAAHC standards
  • Support continuous improvement focused on patient care quality
  • Deliver RN care coordination and clinical support, including assessments, documentation, and blood draws
  • Oversee multiple chronic condition programs to drive patient engagement and outcomes
  • Partner with the Nurse Practitioner on PHAs, wellness events, and engagement initiatives
  • Travel to assigned mini-sites for operational oversight and connection
  • Travel is variable, averaging ~2 travel days per quarter
    • Some periods may include heavier travel (e.g., events), followed by lighter or no-travel weeks
What You’ll Bring

  • Active Compact RN or multi-state licensure
  • Current BLS or CPR certification
  • 5+ years of RN experience
  • Bachelor’s degree in Nursing (strongly preferred)
  • 1+ year of people leadership or team lead experience (strongly preferred)
  • Comfort balancing clinical care with leadership and operational responsibilities
  • Willingness to travel between sites as needed
